Historically, opioids have been the mainstay of therapy for critically ill patients who require analgesia for prevention and management of acute pain, but growing evidence has shown that the short-term use of opioids following surgery in previously opioid-naive patients carries an increased risk of chronic opioid use and is associated with detrimental outcomes. There is also increasing awareness of opioid-related side effects. Various opioid-sparing and opioid-minimizing multimodal pain regimens are emerging, with the potential to reduce exposure to opioids without compromising analgesia. Devising safe and effective therapeutic regimens can be especially challenging in the face of underlying conditions such as organ dysfunction, surgery, or polypharmacy-related adverse events. Multiple protocols for acute pain management include: 1) systemic lidocaine infusion for acute pain management in opioid-naive patients, 2) low-dose ketamine infusion for acute pain management in opioid-dependent surgical patients, and 3) nerve block training for residents and faculty who perform bedside abdominal and chest wall field blocks. This session will focus on the conceptual framework, tools, and literature addressing effective pain management strategies that can improve outcomes and quality of life for critically ill surgical patients. Speakers are pharmacists, physicians, and anesthesiologists, who will share their experiences, including challenges and adaptations.
