Under Pressure: How to Optimize Hemodynamic Resuscitation

Hemodynamic instability is one of the defining features of septic shock. International guidelines recommend an initial attempt to restore perfusion with volume resuscitation, followed by hemodynamic support with vasopressors. The optimum volume of fluid infused, the timing of vasopressors, and the endpoints of resuscitation are clearer than in prior years but still remain uncertain. In this lecture, we will review strategies that hospitals may use to achieve resuscitation more safely and rapidly, including earlier use of peripheral vasopressors, efficient methods to assess volume responsiveness, and empiric fluid choices. Dr. Lesur is director of critical care research at the University of Sherbrooke and lead author of a review on hemodynamic resuscitation in early sepsis choices.
